Overall recruitment flow diagram. Multimedia advertising directed potential participants to tier 1 screening via patient facing web portal eligibility survey. Those meeting primary eligibility criteria were referred to tier 2 screening. Standardized phone assessment of modified Rankin score and level of disability were used to determine appropriateness of obtaining medical records to further screen for eligibility. Those appearing to meet the study inclusion and exclusion criteria were then referred to the nearest study assessment site for formal evaluation and consent at visit 1
