Table 1 Inclusion and exclusion criteria

Have had a previous hospitalisation for heart failure

Unstable angina pectoris

In New York Heart Association-Functional Class II or III

Recent unexplained syncope

Approval from responsible clinician to participate in the Home-Heart-Walk program

Failure to give informed consent

Resting HR greater then 120

Inability to perform 6MWT

Significant cognitive impairment
