¹Self-reported data are collected up to seven times during the first year, or until the patient’s death or study withdrawal, whichever comes first. As final date for inclusion is 31 December 2020, patient follow-up will continue in 2021
²Caregivers receive two forms at 6 and 12 months after the patient is dead, provided that they consent to continue in the study. Thus, caregiver follow-up may continue in 2022
