3 Preventability not quite likely (less than 50/50, but “close call”)
4 Preventability more than likely (more than 50/50, but “close call”)
5 Strong evidence of preventability
6 Virtually certain evidence of preventability
  1. Criteria used to determine potential preventability in EPOCH. These criteria were used in the Canadian Adverse Events Study. A rating of 4 or more will be regarded as a high degree of preventability. Potentially preventable cardiac arrests will be presented as rate per thousand patient-days.
