Inclusion criteria

Exclusion criteria

▪ Average screening systolic BP ≥ 140 mmHg on 2 screening visits within 7 days of each other

▪ Average screening systolic BP ≥ 200 mmHg or average diastolic BP ≥ 120 mmHg (on 3 consecutive visits)

â–ª Recently admitted to the home care post-acute care program

â–ª Dialysisa

â–ª End stage renal diseasea

▪ 21 years of age or older

â–ª Kidney transplanta

â–ª Black and/or Hispanic

â–ª Severe heart failurea

â–ª Speaks English or Spanish

â–ª Significant cognitive impairment. Unable to provide informed consent, accurate self-report, and/or unable to participate effectively in intervention

â–ª History of stroke (ischemic or hemorrhagic) or transient ischemic attack

â–ª HTN diagnosis

▪ Patients with upper arm circumference ≥ 38 cm. At this dimension our blood pressure cuffs become inaccurate

â–ª Resides in the study catchment area

â–ª Has a telephone (in order to participate in intervention phone sessions)
