Select by location (eg cortical, MCA territory, etc)

Select by size (DWI)

Select by perfusion defect (PWI, MRA) for reperfusion therapies

Select by diffusion/perfusion mismatch (DWI, PWI) for neuroprotective

drugs

Exclude if confounding subacute or chronic lesions
