  1. *The primary end-point for data analysis is 6 months post-surgery. All outcome measures will be undertaken pre-surgery, then at 10, 26 and 52 weeks post-surgery, with the exception of return to work and healthcare consumption and related costs, which will not be assessed at baseline.
