A1c Change in the percentage of eligible patients with an average A1c > 8%

6 months preceding and following intervention period

Hierarchical logistic regression with a dichotomous dependent variable indicating whether each patient on a team is in control or not in each of the baseline and post intervention periods. The cluster identifier is the team, and the analysis will be stratified by site with fixed site effects. Using an analogous model, the primary hypothesis will be tested by examining the interaction between the treatment group variable and the time period variable at the laboratory measurement level.
